Client Forms & Helpful Documents
To make your experience as smooth as possible, we provide required and optional forms in advance.
These forms may include:

- HIPAA and privacy notices

- Court-required forms (when applicable)
Completing forms ahead of time helps reduce delays and ensures your appointment is as productive as possible.

Employee Assistance Programs (EAPs)
Braden Counseling Center maintains provider agreements with many national and regional Employee Assistance Programs (EAPs). These services allow employees and their families to access confidential counseling and support services. We are an affiliate provider for:

Most EAP plans offer 3–12 sessions at no out-of-pocket cost and require preauthorization before scheduling
Fees & Payment Options
We strive to make services accessible and transparent. Payment options include:
- Insurance billing (when applicable)
- EAP coverage
- Cash and credit card payments
- Sliding scale fees for eligible clients
- Program-specific fees (such as evaluations or court-mandated services)
Our team is happy to review costs with you in advance so there are no surprises.
Military, Veterans, & TRICARE Services
Braden Counseling Center is proud to support service members, veterans, and their families.We are TRICARE-approved and provide services to veterans and military-connected individuals through approved insurance and EAP programs, including Military OneSource.
Services are confidential, respectful, and designed to address:
- Mental health concerns
- Trauma and PTSD
- Substance use challenges
- Life transitions and stress
Provider Referral Resources
We value collaboration with physicians, attorneys, probation departments, schools, and other community partners. Provider referrals are welcome for:
- Counseling services
- DUI evaluations and treatment
- Substance use and IOP programs
- Domestic violence programs (PAIP)
- Mental health medication management
Our team works efficiently to coordinate care, share documentation when authorized, and maintain clear communication throughout the referral process.
